Care Transitions & Admissions Liaison

Avem Health Partners seeks a Clinical Liaison to assess potential residents in the hospital and determine eligibility for care in collaboration with physicians and the care team. The role focuses on building key relationships with discharge planners, physicians, and other healthcare providers to support placement decisions.
