A-Line Staffing is now hiring a Full-Time, Field-Based, Home Health Case Manager in Northeast, OH!

Candidates must reside in Northeast Ohio , preferably near:

• Trumbull County
• Stark County
• Seneca County
• Portage County
• Mahoning County

We are seeking an experienced Registered Nurse Care Manager to support members through comprehensive care management services. This role focuses on assessing member needs, developing individualized care plans, coordinating services, and providing education and support to help members achieve improved health outcomes and maintain independence.

The ideal candidate is an experienced RN with a background in case management, care coordination, or home health who is comfortable working independently, managing a flexible schedule, and providing compassionate care to members in their homes and communities.

Home Health Case Manager Compensation

• The pay for this position is $42/hr
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Home Health Case Manager Highlights

• This position is a contract assignment with potential for permanent hire based on attendance, performance, and business needs
• The required availability for this position is Monday–Friday, 8:00 AM–5:00 PM

Home Health Case Manager Responsibilities

·         Assess members’ medical, behavioral, and social needs to develop personalized care plans.

·         Coordinate healthcare services and resources to support positive member outcomes.

·         Conduct in-home member visits (approximately 50–60% of the role).

·         Provide education and guidance to members and their families.

·         Collaborate with providers, community resources, and internal teams.

·         Complete required documentation within established turnaround times.

·         Maintain accurate and timely records following each member interaction.

·         Manage daily workload independently while meeting productivity expectations.

·         Participate in team meetings, training sessions, and ongoing professional development.

Performance Expectations

Successful candidates will:

• Complete 5–7 in-home member visits weekly.
• Complete documentation within 24 hours.
• Meet required turnaround times and productivity goals.
• Complete assigned training and compliance requirements on time.
• Deliver high-quality, member-focused care management services.

Home Health Case Manager Requirements

• Active Ohio Registered Nurse (RN) license .
• Degree from an accredited School of Nursing.
• 2–4 years of nursing experience.
• Experience in care management, case management, or home health preferred.
• Reliable transportation for weekly field visits.
• Strong computer skills and ability to work with Microsoft Office applications.
