Compassionate Hospice RN Case Manager

Gentiva Health Services, Inc. seeks a compassionate Hospice RN / RN Case Manager to guide patients and families through the home-based hospice journey. You will deliver skilled nursing care per each Plan of Care in patients’ homes, assess needs, and coordinate with the IDG to ensure comfort, dignity, and continuity. The role emphasizes caregiver education, timely clinical documentation, and collaboration with physicians, LPNs, CNAs, social workers and chaplains to refine care plans. #J-18808-Ljbffr
