Hospice Registered Nurse Case Manager

Optimal Care in Kalamazoo, MI is seeking a Hospice RN Case Manager to provide skilled nursing care and compassionate support to patients and families in their final chapter. You will coordinate comprehensive care for your caseload and work with an interdisciplinary team to address physical, emotional, and spiritual needs. This role offers travel to patients’ homes, facilities, and requires building strong relationships with families and healthcare providers. #J-18808-Ljbffr
