Social Worker: Community Resource Navigator
St. Elizabeth Physicians
St. Elizabeth Physicians in Kentucky is seeking a Licensed Social Worker (LSW) to join the Care Management – Integrative Care Team. The LSW provides population health support by connecting patients to community resources and conducting face-to-face encounters in primary care offices, Medicaid offices, libraries, and patients' homes. You will coordinate DME, transportation, housing, food programs, SSI/SSDI, and Medicaid applications, while communicating with physicians, nurses, care coordinators, #J-18808-Ljbffr St. Elizabeth Physicians
