Our flagship workforce development program, Ready, Willing & Able (RWA), operates across 660 transitional housing beds and combines housing and comprehensive supportive services with paid work training, career preparation, and occupational skills training. The Doe Fund’s innovative social enterprise model creates additional opportunities for New Yorkers to gain paid, hands-on experience while delivering essential services across the city, including supplemental sanitation, graffiti removal, and rain garden maintenance. We are also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 2,100 apartments in operation or development, creating safe, dignified homes with supportive services and spaces that foster community and connection. The Doe Fund has an annual operating budget of $88 million and a full-time staff of over 500, a significant portion of whom are graduates of our programs.

We are seeking a Case Manager for The Doe Fund's newest permanent supportive housing for homeless individuals, including veterans living with a severe mental illness and/or history of substance use. The housing program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 90-unit residence will provide housing for 54 individuals referred by the NYC Human Resources Administration (HRA), and the remaining 36 units will be apartments for affordable to low-income households. 

The Case Manager is responsible for providing supportive case management services and counseling to the residents referred by HRA. The successful candidate will have strong clinical skills and experience working with individuals diagnosed with co-occurring disorders and/or trauma.

Key Responsibilities:

• Maintain a caseload of individuals diagnosed with serious mental illness and/or substance use disorder and provide supportive case management services to them 
• Develop and facilitate therapeutic groups for residents
• Maintain resident records in compliance with agency and funding standards 
• Conduct and review assessments
• Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency 
• Monitor and ensure residents are linked to appropriate treatment and services
• Provide counseling and crisis intervention as necessary
• Assist residents in securing and maintaining benefits
• Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs
• Monitor and assist residents with rent payments and budgeting
• Conduct regular unit inspections 
• Educate residents about approaches to mental health and substance use
• Compile resources on mental health information and treatment strategies
• Attend ongoing training to enhance the services provided to the residents
• Assist with the completion of reports as needed
• Other duties as assigned

The Ideal Candidate:

• You have a Bachelor's degree in Social Work or a relevant field . 
• You have at least 3 years of experience working with homeless individuals diagnosed with serious mental illness and/or substance abuse disorder and/or in trauma settings
• You have expertise in Evidence-Based Practice, including Critical Time Intervention, Motivational Interviewing, and Harm Reduction techniques
• You have excellent clinical and case management skill,s including knowledge of benefits in the NYC Metro area
• You are sensitive to community issues and needs
• You have outstanding interpersonal and communication skills, both oral and written
• You have strong ethical standards, confidentiality, and personal integrity
• You are computer proficient (i.e., email, Microsoft Word, Excel, and PowerPoint)
• Bilingual preferred.
