Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting within an assigned territory
• Manage a caseload of approximately 6–7 patient visits per day
• Evaluate, develop, and implement individualized patient care plans in accordance with physician orders and Medicare guidelines
• Document all patient care activities and progress accurately using the HCHB electronic medical records system
• Collaborate with interdisciplinary care team members to ensure coordinated and effective patient outcomes

Required Qualifications

• Active and unrestricted Physical Therapist (PT) license in the state of California
• Minimum of 1 year of home health experience at a Medicare-certified agency
• Proficiency with HCHB (HomeCare HomeBase) EMR system
• Valid driver's license and reliable transportation for patient home visits within the assigned territory

Preferred Experience / Skills

• Familiarity with Napa County geography and patient population
• Strong time management and organizational skills to efficiently manage a daily patient caseload
