SI PPS Peer Recovery Specialist

Staten Island Performing Provider System (SI PPS) is an integrated network of medical, behavioral health, and social service providers dedicated to transforming care for Staten Island residents. Our mission is to connect Medicaid members to free services, including food, housing, transportation, and other essential resources. Through collaboration and innovation, we strive to improve health outcomes and address the social determinants of health in our community. Job Overview Under the direction of the Clinical Director, the SI PPS Peer Recovery Specialist is embedded within the Staten Island Performing Provider System’s Hotspotting the Overdose Epidemic Program and serves as a bridge between high-risk individuals identified through data analytics and Camelot’s clinical services. The Peer provides harm reduction outreach, rapid engagement, and warm handoffs to care. In addition, the Peer Recovery Specialist will be stationed at Staten Island Performing Provider System (SI PPS), Staten Island’s Social Care Network (SCN) lead entity, to conduct Health-Related Social Needs (HRSN) screenings and support navigation to behavioral health, substance use, and social care services as part of the SCN initiative. Core Responsibilities Provide on-site engagement and support at identified high-risk locations. Conduct screenings using the Accountable Health Communities (AHC) tool. Collaborate with SI PPS care coordinators and Camelot clinicians to ensure follow-up. Support harm reduction education, overdose prevention, and naloxone distribution. Maintain real-time documentation and communication across both systems in accordance with OASAS and agency standards. Participate in case conferences, supervision, staff training, data tracking, and outreach planning. Provide outreach, engagement, and recovery support in community and field-based settings. Assist participants in identifying personal recovery goals and developing individualized recovery plans. Coordinate with Health Homes, emergency departments, and justice-involved services to re-engage individuals lost to care. Educate clients about self-help techniques, relapse prevention, and coping strategies. Facilitate warm handoffs to treatment, housing, medical, and social services. Provide information and connections to mutual aid groups such as NA, AA, and SMART Recovery. Teach self-advocacy and empowerment skills to help clients navigate systems of care. Uphold confidentiality and adhere to ethical guidelines consistent with 42 CFR Part 2 and HIPAA. Model professionalism, effective coping skills, resilience, and the belief that recovery is achievable for all. Screening & Navigation Responsibilities Conduct Health-Related Social Needs (HRSN) screenings using the NYS-standardized tool in collaboration with SI PPS. Complete screenings in community-based settings (e.g., courts, shelters, community organizations, and other partner sites) as assigned. Use designated care‑coordination platforms (e.g., Channels360®) to document screenings, referrals, and follow‑up navigation. Initiate warm handoffs and referrals to social care services (e.g., housing, food assistance, care management), and to behavioral health and substance use treatment providers including Camelot and external partners. Coordinate with SI PPS and Camelot staff to support closed-loop referrals and ensure participants do not get lost to follow-up. Supervision/Reporting Structure The Peer Recovery Specialist is employed by Camelot and reports directly to the Program Director/Clinical Director. For SCN-related activities, the Peer will also work in close coordination with the SI PPS Director of Behavioral Health and the SI PPS Field Operations Manager regarding screening workflows, community assignments, and reporting expectations.
