Home Health Hospice Case Manager

We are seeking a skilled Home Health Hospice Case Manager with experience in the home health setting to provide compassionate end-of-life care. The ideal candidate will possess a strong understanding of hospice principles and demonstrate the ability to deliver high-quality nursing care in patients' homes.

Responsibilities include:

• Conducting comprehensive assessments of patients and developing individualized care plans.
• Coordinating and collaborating with multidisciplinary teams to ensure optimal patient outcomes.
• Providing education and support to patients and their families regarding hospice services.
• Utilizing effective communication skills to ensure clear and ongoing conversations with patients and families.
• Monitoring patient progress and modifying care plans as necessary.
• Documenting all nursing assessments, interventions, and patient progress accurately in compliance with relevant standards.

Qualifications include:

• Current licensure as a Registered Nurse (RN).
• Experience in hospice care within a home health setting is required.
• Strong assessment and critical thinking skills.
• Excellent interpersonal and communication abilities.
• Ability to work independently and manage a caseload effectively.

Additional information includes:

• Day shift available, 5 days a week (08:00 - 16:30).
• Mileage will be reimbursed at the IRS rate.
• No on-call responsibilities.
