Growing together.As the Hospice Nurse Practitioner, you will provide consultation in palliative care, symptom management and supportive care to meet the needs of the Hospice patient as requested by the referring physician.Primary Responsibilities:Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical DirectorConsults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as neededInitiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to hospice care. Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family careInstructs and supervises the patient/family in self-care techniques when appropriateMaintains accurate and relevant clinical notes regarding the patient's condition. Reports patient condition and any identified needs/issues to Administrator and Hospice Medical DirectorYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Active unrestricted license in the state of residenceCurrent certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practiceCurrent certification by a national body such as ANCC, NBCHPN, or AANP5+ years of nursing experienceCurrent driver's license and vehicle insurance, access to a dependable vehicle or public transportationPreferred Qualifications:Current CPR certification or ability to complete within 90 days of hireExperience working with an interdisciplinary teamClinical nursing care in oncology, hospice, geriatrics, or intensive carePalliative Care ARNPPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $43.58 to $65.37 per hour based on full-time employment.
