Physical Therapist - Per Diem

Role Overview A PRN Physical Therapist in Home Health conducts assessments and evaluations to determine patients' functional and mobility needs. They develop and implement treatment plans to improve patient well-being, monitor progress, and collaborate with healthcare team members. Key Responsibilities Assess patient's functional status and assist in developing therapy plans Provide therapy services within legal scope and coordinate with the healthcare team Monitor and report patient responses to treatments and condition changes Treat patients to relieve pain, restore function, and maintain maximum functioning Educate patients, families, and healthcare team members about care plans and progress Complete patient evaluations, develop plans of care, and document visits and outcomesAdapt to diverse home environments and patient backgrounds Qualifications & Skills Current Physical Therapy license in the practice state Valid driver's license, vehicle insurance, or access to transportation Preferred: CPR certification or ability to obtain within 90 days This role offers competitive compensation based on productivity and performance, along with benefits such as health coverage, recognition programs, and retirement options.
