Hospice Triage Nurse

Role: Hospice RN On Call Triage/Float Responsibilities Provide after-hours phone triage for hospice patients in their homes Conduct occasional field visits to assess patient conditions Assess symptoms, prioritize care needs, and coordinate with the interdisciplinary team Support families during critical moments with compassionate, evidence-based care Ensure timely interventions and document care in the EHR Float to support in-person visits as needed Qualifications & Skills Registered Nurse (RN) license Strong assessment and triage skills Excellent communication and compassionate patient care abilities Ability to work independently and collaboratively Comfort with documentation and electronic health records Key Qualities: Dedicated to quality, integrity, and patient-centered care, with opportunities for growth and training within a collaborative environment. #J-18808-Ljbffr CARIN Alliance
