Hospice RN Case Manager Lead Care Team
Priority Dispatch Corp.
Kindful Health is seeking an RN Case Manager to lead a care team and provide best-in-class hospice care for patients and families in Texas. You will collaborate with LPNs, aides, MSW and a chaplain to manage a reasonably sized census and ensure coordinated care. You will present biweekly at IDT and work with the Clinical Director and Medical Director to maintain high standards and a concierge hospice experience.
