Nurse Practitioner for In-home Visits
$2,500 - $4,000 per weekSignify Health
Job Overview Signify Health, part of CVS Health®, is hiring board‑certified nurse practitioner contractors to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Responsibilities Conduct in‑home visits focused on a member’s overall health, well‑being, and potential barriers to care. Perform medication and medical history review and a physical evaluation. Complete a pre‑populated, patient‑specific form for each assessment. Do not prescribe medication, order lab tests, or alter existing treatment regimens. Compensation Earn up to $2,500 to $4,000 each week (average 6‑9 visits daily, base rate $80 per completed assessment). Increase per‑visit rate up to 135% depending on working counties (Market Multiplier). Earn additional compensation via a pay‑per‑test model for qualifying diagnostic and preventive tests. Availability incentives based on weekly availability. *Incentive rates are subject to change. Area exclusions apply. Benefits Shifts available Monday‑Sunday (no nights or weekends required). Short‑term or long‑term basis. Pre‑scheduled visits in the member’s home. Airfare, hotel, and rental car available (depending on eligibility). Malpractice coverage. Independent Contractor status (1099). Flexible schedule. Qualifications & Requirements Board‑certified nurse practitioner. State license required. Ability to travel to member residences; no night or weekend shifts required.
