Bond Community Health Center, Inc. is seeking a compassionate, organized, and community-focused Outreach And Enrollment Specialist to support the organization's efforts to increase access to healthcare services and affordable health coverage.

The Outreach And Enrollment Specialist assists individuals and families with understanding available healthcare coverage options, completing eligibility and enrollment processes, and connecting with appropriate community resources. This position conducts both "in-reach" activities with existing BCHC patients and "outreach" activities with individuals who are not currently BCHC patients throughout the communities served by Bond Community Health Center.

The successful candidate will demonstrate excellent communication skills, cultural sensitivity, attention to detail, confidentiality, and a genuine commitment to serving underserved and diverse populations.

Essential Duties And Responsibilities

• Respond promptly and professionally to requests for assistance related to Mobile Health Services and Outreach.
• Provide information regarding healthcare coverage and available programs in a fair, accurate, impartial, and understandable manner.
• Maintain current knowledge of applicable eligibility requirements, enrollment procedures, program specifications, and available coverage options.
• Assist individuals with the application and enrollment process for available healthcare coverage and assistance programs.
• Accurately collect, enter, maintain, and report required enrollment and outreach data.
• Conduct "in-reach" activities with BCHC patients and "outreach" activities with non-BCHC patients throughout designated service areas.
• Monitor and report patient and non-patient enrollment activity, including completed applications and enrollments.
• Provide educational materials and enrollment information to community partners, including health departments, hospitals, urgent care centers, physician offices, human services agencies, and other referral organizations.
• Develop and maintain effective working relationships with BCHC providers, staff, community organizations, referral sources, and other partners to support outreach and enrollment objectives.
• Assist with planning, coordinating, and implementing Mobile Health Services and Outreach activities and community events.
• Participate in outreach events that may occur before or after normal business hours, including evenings and weekends, as needed.
• Assist with presentations and educational sessions for community groups, referral sources, patients, and other stakeholders.
• Assist with the development and distribution of promotional and educational materials appropriate for the literacy and informational needs of the intended audience.
• Attend and successfully complete all required federal, state, local, and BCHC training sessions related to eligibility, enrollment, outreach, and program requirements.
• Attend meetings and participate in activities related to Mobile Health Services and Outreach.
• Safeguard protected and confidential information and comply with all applicable privacy and confidentiality requirements.
• Maintain accurate documentation and complete required reports within established deadlines.
• Organize and prioritize work effectively to meet departmental goals, objectives, and deadlines.
• Manage multiple assignments while maintaining accuracy and quality of work.
• Work independently while also contributing effectively as a member of a multidisciplinary team.
• Demonstrate professionalism, compassion, respect, and cultural sensitivity when interacting with patients, community members, coworkers, and community partners.
• Travel throughout the service area as required to support outreach activities and community events.
• Perform other duties and responsibilities as assigned.

Minimum Qualifications

• High school diploma or equivalent.
• Experience working with the public, preferably in a healthcare, social services, community outreach, enrollment, customer service, or human services environment.
• Demonstrated ability to work effectively with underserved, vulnerable, and diverse populations.
• Strong interpersonal, verbal, and written communication skills.
• Demonstrated compassion, professionalism, and commitment to helping individuals access healthcare and community resources.
• Ability to understand and communicate eligibility, enrollment, and program information accurately.
• Ability to organize and coordinate activities, establish priorities, meet deadlines, and manage multiple responsibilities.
• Ability to work accurately and independently while also functioning effectively as part of a team.
• Ability and willingness to complete all required federal, state, local, and BCHC training related to eligibility, enrollment, and program requirements.
• Ability to maintain strict confidentiality and safeguard sensitive patient and applicant information.
• Valid driver's license, dependable transportation, and willingness and ability to travel as required.

Preferred Qualifications

• Previous experience in healthcare, federally qualified health center (FQHC), public health, social services, community health, health insurance enrollment, or community outreach.
• Experience assisting individuals with healthcare coverage applications or enrollment.
• Experience conducting community outreach, health education, presentations, or enrollment events.
• Bilingual or multilingual skills are a plus, particularly when serving diverse populations.

Work Environment And Schedule

This position may require work in both office and community-based settings, including mobile outreach locations and community events. The employee must be available to work occasional evenings, weekends, or extended hours based on outreach and enrollment activities.

Regular travel within the BCHC service area is required.
