What We Offer $7,500 Sign-On Bonus Competitive pay, benefits, and incentives Full-time position Flexible scheduling with a focus on work/life balance Daily Pay option available No overtime required 1:1 patient care Supportive clinical team Meaningful work that allows you to build lasting relationships with patients and families What You’ll Do as an RN Case Manager: Conduct in-person patient interviews and comprehensive nursing assessments Develop, implement, and continually evaluate individualized patient plans of care Oversee and coordinate care provided by HHAs, LPNs, RNs, and other caregivers Communicate patient conditions and collaborate with physicians and other healthcare providers as patient needs change Educate patients and families on home health care procedures, including wound care, IV administration, and medication management Help reduce hospital readmissions through preventative care and continuity of care Monitor patient progress and make appropriate updates to the plan of care What You’ll Need: Current, unrestricted RN license in the state associated with this position Current CPR/AED/BLS/First Aid certification Reliable transportation to and from patient care sites At least 1 year of professional RN experience in home health or a similar setting Previous case management or utilization review experience preferred OASIS experience preferred Practical tracheostomy and/or ventilator experience preferred, but not required Why Interim HealthCare?
