Social Worker - MSW Profession: Social Services Specialty: LMSW (Licensed Master of Social Work) Duration: 2 years Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: 1 year of medical-surgical discharge planning required License: Washington State Licensed required Certifications: LMSW Must-Have: Flexibility to travel within King County Description:This position is for a full-time social worker engaged in a state pilot program.The focus will be on transitioning complex patients from the hospital to the community.Candidates must be willing to work in inpatient, outpatient, and community settings.Experience in both hospital and community mental health settings is preferred.The ability to travel within King County is required, with the majority of travel planned.Travel may include visits to nursing facilities, adult family homes, and other patient residences.Travel is expected to occur 2 to 3 times per week.Candidates must have strong fluency in electronic medical record systems, particularly Epic.There is a potential requirement for one pre-planned weekend shift per month.Flexibility and strong interpersonal skills are essential for this role.Experience with discharge planning and care coordination is crucial.The role involves assessing patient needs and developing appropriate care plans.Collaboration with interdisciplinary teams to facilitate patient care is expected.Documenting patient response and maintaining accurate records is a critical responsibility.Knowledge of federal, state, and county health regulations is beneficial. #J-18808-Ljbffr Cynet HealthStaff
