Key Responsibilities\n

• Conduct comprehensive bio-psychosocial assessments to evaluate patients' conditions, identifying barriers to discharge, and maximizing patient care.

• Utilizing evidence-based treatment methodologies, develop individualized treatment plans aimed at improving patients’ mental health and overall psychosocial functioning.

• Collaborate with a multidisciplinary team to design and implement integrated care strategies that ensure seamless treatment transitions and enhanced patient satisfaction.

• Document all clinical interactions and treatment activities in compliance with departmental standards, ensuring accurate record-keeping to facilitate proactive care.

• Provide psychoeducation to patients and their families, assisting them in understanding and utilizing resources for both medical and social support.

• Assess and coordinate advocacy needs for patients within the community to enhance their social support systems and resource accessibility.

• Stay informed on local community resources and engage with outreach initiatives that empower clients to achieve greater self-determination.

Patient Population / Client Focus\n

In this role, you will primarily work with patients undergoing detoxification and recovery from substance use disorders. Qualifications\n

• A Master's Degree in Social Work from an accredited institution, alongside a current Licensed Clinical Social Worker (LCSW) license issued by the State Board of Social Work Licensure.

Compensation and Benefits\n

The compensation for this position ranges from USD 85,000 to 100,000 annually, commensurate with experience.
