Registered Nurse - Wound Care

Job Summary:

The Wound, Ostomy, and Continence Nurse is responsible for delivering specialized, evidence-based care to patients with wounds, ostomies, or continence issues. Reporting to the Wound and Ostomy Manager, this role involves comprehensive assessments and collaborative treatment planning to promote healing and prevent complications.

Responsibilities:

• Conduct detailed health assessments focused on wound, ostomy, and continence conditions.
  
• Develop and adjust personalized plans of care based on patient needs and clinical findings.
  
• Educate and counsel patients and families on wound management, ostomy care, and prevention strategies.
  
• Monitor and document patient responses to treatments, adjusting care plans as necessary.
  
• Collaborate with interdisciplinary teams and refer to specialists when needed.
  
• Participate in quality improvement initiatives and document outcomes to support clinical excellence.
  Qualifications:
  
• Active NYS Registered Nurse license.
  
• Certified Wound Ostomy Nurse (CWON), Wound Care Certified (WCC), CWCN, or CWS credential.
  
• Three to five (3-5) years of healthcare experience with direct wound or ostomy care.
  
• Proficient in utilizing electronic health record systems and healthcare software.
  
• Strong interpersonal and patient education skills.
