365 Healthcare is recruiting a Board Certified Family Medicine Physician for a full-time permanent outpatient opportunity in Saratoga Springs, New York.

Position Snapshot

• Base Salary: $250,000 - $260,000 + productivity incentives
• Sign-On Bonus: Up to $75,000
• Retention Bonus: $25,000 (Year 2)
• Schedule: 4- or 5-day work week
• Hours: 40 hours/week (34 patient-facing)
• Patient Volume: 18-24 patients per day
• Employment Type: Full-time, permanent

Schedule & Work-Life Balance

• Flexible 4- or 5-day schedule
• No in-office weekend hours
• Call: Approximately 1 in every 70 days
  • Phone call only
  • RN triage handles first contact
• Telemedicine coverage:
  • 2-3 hours on a weekend once every 3 months

Practice Environment

• Established outpatient primary care practice
• Physician-led team-based care model

Clinical Responsibilities

• Provide comprehensive primary care to patients of all ages
• Manage chronic and acute conditions
• Perform routine exams and preventive care
• Collaborate with APPs and care teams
• Maintain accurate documentation in Epic
• Deliver patient-centered, evidence-based care

Qualifications

• Board Certified or Board Eligible in Family Medicine
• Active New York license (or ability to obtain)
• DEA and ACLS certification (by start date)
• Strong interest in full-spectrum primary care
• Clean malpractice and license history
