As a Wellness Nurse LPN, you play a central role in monitoring resident health, supporting medication oversight, and maintaining clinical quality across the community. What You'll Do: Conduct move-in, 3-day, semi-annual, and change-of-condition assessments Perform monthly wellness visits and monitor health trends Follow up on falls, incidents, and hospital returns Transcribe physician orders and ensure accurate MAR/TAR documentation Complete medication audits and oversee medication compliance Partner with families, physicians, and the Resident Care Director Support infection control and regulatory standards Educate and guide care staff on documentation and medication practices You help prevent decline through proactive assessment, early intervention, and strong clinical oversight.What We're Looking For: Active LPN license in good standing Graduate of approved nursing program 1+ year experience in senior care or clinical setting Strong assessment, documentation, and communication skillsLicensed Practical Nurse (LPN/LVN) – Support Resident Health and Wellbeing Licensed Practical Nurse LPN LVN | Nursing | Assisted Living | Senior LivingPay Range: 30.00 - 40.00Ready to take the next step and make a bigger impact than you ever imagined?
