Overview

Responsibilities
- Conduct comprehensive occupational therapy evaluations for adult residents and develop individualized treatment plans focused on activities of daily living and functional mobility.
- Implement and adjust OT interventions to improve independence, safety, and quality of life; document progress and functional outcomes.
- Collaborate with an interdisciplinary team (nursing, PT, speech therapy, case management) to support coordinated resident care and discharge planning.
- Monitor resident progress, reassess goals, and modify treatment plans as needed; educate residents and families on home programs and strategies to maintain gains.
- Ensure compliance with regulatory requirements and facility policies; maintain accurate records and timely documentation in the electronic health record.
- Participate in care conferences and quality improvement activities to support optimal resident outcomes.

Requirements
- Active occupational therapy license in the United States; state-specific licensure required or eligibility for licensure in the state of New Mexico.
- Preferably 1 year of OT experience in a general or rehabilitation setting, with mileage toward SNF/LTC experience.
- Ability to work in a skilled nursing facility environment and collaborate with a multidisciplinary team.
- Strong documentation skills and proficiency with electronic health records.
- Must be able to travel to the assignment location and commit to the stated shift and hours.
