Primary Care Nurse Practitioner or Physician (MD/DO) – Bronx, NY

Part-Time Opportunity | 12–16 Hours Per Week | Assisted Living Setting

Visiting Docs is currently seeking an experienced Nurse Practitioner or Physician (MD/DO) to provide onsite primary care services within assisted living communities located in the Bronx, NY.

This is an excellent opportunity for a provider looking for a flexible part-time schedule while working with an established healthcare organization focused on delivering quality care to the geriatric population.

Position Overview

• Part-time schedule
• Approximately 12–16 hours weekly
• Weekday daytime hours only
• No overnight shifts
• No weekend requirement for this role
• Flexible scheduling available based on provider availability and facility needs

Clinical Responsibilities

• Perform routine and follow-up primary care visits
• Evaluate and manage chronic and acute medical conditions
• Prescribe medications and treatment plans
• Coordinate care with nursing staff, specialists, and families
• Complete documentation in the electronic medical record system
• Provide patient-centered care to the assisted living population

Minimum Qualifications

• Active New York State NP or Physician license
• Active DEA registration
• Current Medicare and Medicaid enrollment required
• Ability to practice independently
  • Nurse Practitioners must meet NYS independent practice requirements (3,600 practice hours)
• Prior primary care experience

Preferred Experience

• Geriatrics
• Assisted living or long-term care
• Strong communication and organizational skills

Important Information

Candidates must already be actively enrolled with Medicare and Medicaid. Due to lengthy processing timelines, we are unable to consider providers who are only eligible to enroll or who have not yet completed the enrollment process.
