Facility: Beach Breeze Rehab & Care Position: Social Services Director Location: West Palm Beach, Florida Schedule: Full-Time | Monday–Friday, 8:30 AM – 5:00 PM Salary: $65,000 – $85,000 annually Join Our Team as a Social Services Director! Beach Breeze Rehab & Care is seeking an experienced and compassionate Social Services Director to lead our Social Services Department. This key leadership role focuses on supporting residents and families while ensuring compliance, advocacy, and high-quality service delivery in a skilled nursing environment. Key Responsibilities Lead and oversee the Social Services Department to ensure exceptional resident support and service delivery Conduct comprehensive resident assessments and develop individualized care plans Support residents and families through the admission and transition process Develop, implement, and maintain departmental policies and procedures Identify and resolve operational issues promptly and effectively Continuously evaluate resident needs and maintain open, ongoing communication with families Provide solutions for social, emotional, environmental, and financial challenges, including referrals to community resources Collaborate closely with interdisciplinary teams to deliver holistic resident care Represent the facility during regulatory surveys and inspections, ensuring full compliance Serve as a liaison between residents, families, and medical staff, advocating for residents’ social, emotional, and psychological well-being Requirements Minimum of 3 years of social services experience , preferably in a Skilled Nursing Facility (SNF) Strong interpersonal, communication, and problem-solving skills Ability to lead with empathy, professionalism, and attention to detail Benefits Generous Paid Time Off (PTO) Medical and Dental Insurance Company Perks Employee Recognition ProgramsSupportive, team-oriented work environment Ready to Make a Difference?
