Pet insurance. Responsibilities Evaluate patients to determine rehabilitation needs, potential for achievement, and develop a plan of care in accordance with findings. Participate in the implementation and development of the Plan of Care to ensure quality and continuity of care, proper discharge planning, and compliance with OASIS and PPS regulatory guidelines. Communicate significant findings, problems, changes in condition or environment to the appropriate supervisor, physician, and other team members involved with patient care. Initiate plans for patient safety using patient, family, and community resources. Verify the Plan of Care prior to each visit and provide care according to physician’s orders, assessment data, and established standards and guidelines. Write physician orders to cover additional visits and changes to the Plan of Care, per agency policy. Incorporate patient care goals from the Plan of Care into therapy sessions, as documented in therapy notes. Perform occupational therapy services in accordance with accepted standards of practice and as certified by the patient’s physician. Instruct patients and/or their families with “activities of daily living” utilizing adaptive equipment, sensory integration, and fine‑motor coordination skills. Assist patients and/or caregivers in securing necessary equipment, including fabrication of custom‑made splints. Instruct the patient and/or caregiver in the therapy home program and provide modification and clarification when necessary. Counsel and instruct patients and family in meeting therapy and related needs. Supervise and provide clinical direction to a Certified Occupational Therapy Assistant (COTA), Home Health Aide (HHA), and other clinical staff to ensure the therapy care plan is followed and quality of care is provided. Physically demanding, high‑stress environment. Full range of body motion, including handling, lifting, and transferring patients. Potentially work irregular hours, including call hours if applicable. Qualifications Graduate of an accredited Occupational Therapy program. Current, unrestricted OT license valid for the state of work. Minimum one (1) year of experience in a clinical care setting. Able and willing to travel within branch/office coverage area. Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. Must be able and willing to travel 50% of the time. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment. You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee. Compensation ranges from $60.00 to $75.00 per point, with additional earnings, on‑call, or other performance‑based compensation available.
