Per Diem and Part time Hospice Registered Nurse

RN – Hospice at Home We’re Hiring an RN for Hospice at Home with AdvisaCare. This is not rushed care but presence, compassion, and honoring each moment. Responsibilities Provide one‑on‑one care in the comfort of home. Support patients and families with dignity and heart. Spend real time where it matters most, caring without the chaos of hospital halls. Conduct supervisory visits to observe clinical performance of team members. Participate and provide feedback in interdisciplinary group meetings, and contribute to development, review, and revision of patient/family plan of care. Provide emotional support to help patients live their final days with dignity and comfort. Prepare clinical and progress notes, and participate in IDG conferences. Participate in hospice performance‑improvement program. Maintain high level of customer satisfaction with clients, families and caregivers. Communicate information using current process and technology within the organization. Requirements Current Michigan RN license in good standing. Minimum 1 year hospice experience. Empathetic approach to patients and caregivers. Ability to adapt to patient's status and needs on a full‑time on‑call basis. Initial and ongoing assessment of patient's physical, functional, psychosocial and environmental needs. Implement the individualized plan of care and revisions as needed. Compliance with AdvisaCare Hospice policies and procedures and Medicare compliance. Consulting with and educating patient/family on disease process, self‑care techniques, end‑of‑life care, and issues of ethical concern. Supervision of paraprofessionals providing services to patients. Participate in field visits daily, weekly office meetings as indicated. Current on‑call hours 5 pm–8 am; position requires on‑call responsibilities on rotating schedule. Benefits 401 K retirement plan. Medical benefits effective now. Ability to earn PTO.
