Job Description: Licensed Practical Nurse (LPN) – Home Health Shift: Day – 5x8 hours (08:00 AM – 05:00 PM, Monday–Friday) Location: Douglas, GA (Primary Clinic: 8324 – 101 N Peterson Ave) Agency: Amedisys Home Health Key Responsibilities Deliver home health nursing care per physician orders Perform: Start of Care visits Wound care Monitor patient condition and report changes Maintain accurate documentation within 24 hours using EMR Coordinate care with interdisciplinary team Supervise and evaluate: Home Health Aides LPN/LVNs (if applicable) Participate in weekly office meetings Travel Requirements Daily travel between patient homes Coverage territory: Bacon Coffee Counties Travel radius: Up to 75 miles from clinic Daily mileage: 30–40 miles Paid between patient homes only Commute to first/last patient not reimbursed Required Qualifications Experience Minimum 1 year of Adult Home Health OR Hospice experience Must have recent specialty experience (no first-time travelers) Certifications BLS (CPR) required Licensure Active LPN license (must be in hand) Candidate Requirements Must have: Reliable vehicle Valid driver’s license Must be comfortable with: Driving between patients (can exceed 20 minutes per trip) Mandatory proficiency in: HomeCare HomeBase (HCHB) (used within last 6 months) Familiarity with: Outlook (RNs only requirement: OASIS proficiency – not applicable to LPN) *Offered pay rate will be based on education, experience, and healthcare credentials.
