Requirements Expected start date First day should always be reserved for orientation tasks: ASAP – Ideally by Mid-October Specialty: Cardiology FTE needed: 1.0 want to fill with 1 locum provider but open to 2 if they work opposite of each other 1 week on 1 week off or 2 weeks on 2 weeks off Daily Shift Standard 8hr or 12hr: 8 hours Expected duration of assignment: 6 months Reason for opening: Gap in coverage Type of privileges clinic/hospital/both: Clinic Setting: Outpatient Beds: 420 Call requirement: None Rounding admissions or none: None Patient demographics: Adults Average patients per day: 16 Required Procedures: None Required clinical activity in the last 24 months case logs: Applicant must provide documentation case list of provision of clinical services 100 cases representative of the scope and complexity of the privileges requested during the previous 24 months. Will provider work solo? If not other providers specialties working with them: They will work with our multidisciplinary team- EP Interventionalist APC's RN's MA' Cardiac Sonographers Device Tech's Licensed Candidate: Yes/No: Washington license is preferred but we are open to IMLC with LOQ in hand.
