LMSW - Case Manager, Social Work - Inpatient (Full-Time)

Summary This role involves collaborating with healthcare teams to coordinate cost-effective, appropriate services for members, aiming to improve independence and quality of life. Responsibilities Conduct psychosocial assessments. Develop and monitor care plans. Facilitate access to community resources. Provide supportive counseling to members, families, and caregivers. Manage caseloads. Document activities. Analyze outcomes to improve programs. Assist with applications for community and government programs. Inpatient duties may include completing assessments and guardianship paperwork. Qualifications Master's degree in social work. At least one year of clinical social work experience. Licensure in relevant states within six months of hire. Preferred Skills Proficiency with Windows-based software. Knowledge of community resources.
