Provide high-quality 1:1 nursing care in patients' homes
Assess patient needs and implement individualized care plans
Deliver treatments and skilled nursing services within the RN scope of practice
Monitor patient progress and communicate changes to the care team
Maintain accurate, timely, and compliant clinical documentation
Follow all company policies, clinical protocols, and regulatory requirements
Ensure HIPAA compliance and protect patient confidentiality
Required Skills Qualifications
Required
Active, unrestricted Registered Nurse (RN) license in Illinois OR a Compact Multi-State RN License valid in Illinois
Minimum of 1 year of nursing experience
Active Drivers License, reliable transportation and current auto insurance
Ability to pass background screening
Knowledge of state and federal healthcare regulations
Strong clinical, communication, and critical thinking skills
Preferred
3-5 years of clinical nursing experience
Previous experience in home health, primary care, or a healthcare facility
Bachelor's Degree in Nursing (BSN)
Graduate of an accredited nursing program
Physical Requirements
Ability to stand, walk, bend, squat, kneel, reach, push, pull, and lift during patient care activities
Ability to lift up to 50 pounds with or without reasonable accommodation
Visual and hearing acuity sufficient to safely perform nursing duties
Schedule
PRN and Part-Time hours
Flexible scheduling based on patient needs and nurse availability
