Position Overview
Sakin LLC is seeking a reliable, compassionate, professional, and active Respite Care Provider to provide respite services to a 34-year-old female member in her home and community.

The purpose of respite care is to provide the member's primary caregiver/family with a temporary break from their caregiving responsibilities while ensuring that the member receives safe, respectful, meaningful, and appropriate support.

This is an active caregiving position. The provider is expected to actively engage with and support the member throughout the scheduled shift. This position is not a sitting or waiting position.

The ideal candidate will be patient, dependable, energetic, respectful, and comfortable working directly with a DDD member while following the member's approved service plan and Sakin LLC policies.

Schedule
The current schedule is:

Wednesday: 4:30 PM – 8:00 PM
Friday: 4:30 PM – 8:00 PM
Saturday: 10:00 AM – 10:00 PM
Sunday: 10:00 AM – 10:00 PM

Total scheduled hours: 31 hours per week.

The schedule may be adjusted based on the member's needs, authorized service hours, and the family's availability.

Primary Responsibilities
The Respite Care Provider will:

Provide direct, attentive, and compassionate respite care to the member.
Maintain the member's safety and well-being throughout the entire shift.
Actively engage the member in appropriate activities rather than simply sitting or waiting.
Assist the member with activities that are appropriate to her approved service plan.
Encourage independence and participation whenever appropriate.
Participate in appropriate recreational, social, household, or community activities with the member as permitted by the service plan.
Follow the member's Individual Support Plan (ISP), service plan, and any applicable behavior or support plans.
Follow instructions provided by Sakin LLC and the member's authorized support team.
Respect the member's dignity, privacy, rights, choices, and personal preferences.
Communicate respectfully with the member and her family.
Monitor for changes in the member's needs, behavior, safety, or condition and report concerns to the appropriate Sakin LLC supervisor.
Complete required service documentation and notes accurately for each shift.
Follow all Sakin LLC policies and Arizona DDD requirements.
Report incidents, injuries, suspected abuse, neglect, exploitation, or other concerns immediately according to company policy and applicable requirements.
Maintain appropriate professional boundaries while working in the member's home.
Protect confidential member information at all times.
Active Respite Expectations
The caregiver is expected to be actively providing services during the scheduled hours.

Examples of appropriate activities may include:

Engaging the member in conversation and social interaction.
Participating in games, hobbies, crafts, music, or other preferred activities.
Supporting appropriate recreational activities.
Assisting the member with community activities when authorized.
Encouraging appropriate independence and decision-making.
Supporting the member's daily routine as outlined in her service plan.
Providing companionship and meaningful interaction.
Assisting with appropriate activities of daily living when included in the member's authorized services.
Helping create a safe, positive, and respectful environment.
The caregiver should not spend the shift sitting on their phone, watching television, sleeping, or otherwise disengaging from the member. The provider must remain attentive and available to provide the authorized respite service throughout the scheduled shift.

Qualifications & Requirements
Applicants must:

Be at least 18 years of age.
Have a high school diploma/GED or meet Sakin LLC's applicable hiring requirements.
Have experience providing caregiving, direct support, respite, personal care, or similar services preferred.
Experience working with adults with developmental disabilities is preferred.
Be dependable and able to consistently work the scheduled shifts.
Demonstrate patience, professionalism, good judgment, and compassion.
Be able to communicate effectively in English.
Be able to work independently while following the member's service plan and company instructions.
Be willing to complete all required Sakin LLC and Arizona DDD training.
Successfully complete required background screening and employment requirements.
Provide required references.
Maintain required certifications and training while employed.
The member is a 34-year-old female, and the family has expressed a preference for a female caregiver.

Required Training & Credentials
Before providing services independently, the caregiver must meet applicable Sakin LLC and Arizona DDD requirements.

These may include:

CPR certification
First Aid certification
Article 9 training
Fingerprint clearance/background requirements
Required reference checks
Required DDD/HCBS training
Additional training identified by the member's ISP or service needs
Any other training required by Sakin LLC or Arizona DDD for the authorized service
Sakin LLC will verify the caregiver's credentials and determine which additional training must be completed before the caregiver begins providing services.

Professional Standards
The selected caregiver must:

Arrive on time and remain for the entire scheduled shift.
Notify Sakin LLC as soon as possible if unable to work.
Maintain professional conduct inside the member's home.
Never leave the member unattended when supervision is required.
Never use inappropriate language or behavior around the member.
Never engage in physical, verbal, emotional, or financial abuse.
Never use the member's personal information for personal purposes.
Follow all confidentiality and privacy requirements.
Treat the member with dignity, respect, and kindness.
Follow all applicable Sakin LLC and Arizona DDD policies.
