RN - Care Coordinator

Director Of Care CoordinationWe are seeking a temporary Director of Care Coordination. The qualified candidate must meet the following qualifications. A BSN is required, and a master's degree in nursing, Health Administration, or Business Administration is preferred. Case Management certification (e.g., CCM or ACM) is preferred. The candidate must have a minimum of five (5) years of experience in an acute healthcare setting. The candidate must also have a minimum of five (5) years of progressively responsible leadership/administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function. Demonstrated experience in leading interdisciplinary clinical and operational programs is essential. Experience in managing staff, performance improvement, and regulatory or accreditation readiness is required.
