Physical Therapist

Physical Therapist Home Care (PT) – Queens, NY We are looking for a committed physical therapist for a fee‑for‑service position who can cater patients in Queens and act as the patient’s partner throughout the journey of restoring motion and ensuring that the client will function at their personal best. You will maximize the quality of life and movement potential within the spheres of promotion, prevention, diagnosis, treatment or intervention, and rehabilitation. Responsibilities Provide exceptional physical therapy to patients by assessing and understanding care plans and creating physical‑therapy treatment plans alongside physicians or by prescription. Document visits and updates on patient care and progress. Educate caregivers and their families on how to care for the patient. Make the best judgment on the types of care and services needed for each patient. Support the company mission by providing the highest quality of home care. Requirements Proven work experience as a physical therapist. Graduate degree or greater in physical therapy. Valid license to practice physical therapy. At least one year of experience as a physical therapist in home care, including Medicare Part A experience. Reliable transportation. Excellent communication, organizational, and time‑management skills. Problem‑solving, interpersonal, and computer skills. Ability to work independently with minimal supervision. Experience using HomeCare HomeBase (HCHB) is a plus but not required. Bilingual skills are a plus.
