Step into a dynamic per diem position as a Nurse Practitioner specializing in home health risk evaluations. This role involves traveling to various homes within Madison County, assessing Medicare Advantage plan members in locales such as Granite City, Edwardsville, and Alton. Each comprehensive assessment lasts approximately 45 minutes and may range from annual wellness exams to post-discharge evaluations. Flexible scheduling empowers you to craft your own hours, offering significant autonomy and balance.

Schedule/Hours:

• Flexible shifts available Monday through Sunday
• Freedom to set your weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform thorough annual home health risk assessments for Medicare Advantage members
• Review and document medical history, existing medical conditions, and medication profiles
• Educate patients regarding diagnoses and effectively explain prescribed medications
• Recognize potential care gaps and make actionable recommendations to address them
• Note: No requirement to develop or manage ongoing treatment plans and no medication prescribing

Qualifications:

• Valid Nurse Practitioner License in Illinois
• Active BLS (CPR) certification
• Board Certified through the AANP or ANCC
• Minimum of 6 months' recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
