Job Description

Deliver high-quality care as a Nurse Practitioner specializing in home health services. This per diem role calls for experienced professionals to conduct risk assessments for Medicare Advantage plan members in various locations within Whiteside County, Illinois, such as Sterling, Rock Falls, and Morrison. Visits typically last around 45 minutes and may include annual health assessments or post-discharge evaluations.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Choose your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform home health risk assessments for Medicare Advantage health plan participants
• Review and evaluate prior/current medical histories, ongoing medical conditions, and medication regimens
• Educate members about their health conditions and prescribed medications
• Identify potential gaps in care and offer evidence-based recommendations
• No requirements for initiating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in the State of Illinois
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Six months or more of Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
