Supported Employment Specialist

About the Role: BARYEH HOMECARE SOLUTIONS LLC is looking for a passionate and dedicated Supported Employment Specialist to join our team in Louisville, KY. In this rewarding role, you'll make a meaningful difference by helping individuals with disabilities or barriers to employment find and maintain fulfilling work opportunities. Responsibilities: Work with the Kentucky Office of Vocational Rehabilitation (OVR) and other agencies to support participants in employment. Assist participants in developing person-centered plans for employment success. Provide job readiness training, including application assistance, resume building, and interview preparation. Partner with employers to identify, develop, and maintain employment opportunities for participants. Conduct job site coaching, task analysis, and ongoing support to ensure job retention. Maintain accurate records and case files following organizational and regulatory standards. Collaborate with case managers, referral agencies, and team members to provide holistic support. Participate in professional development activities and maintain required certifications. Requirements: Bachelor's Degree or High school diploma or GED (required). Minimum of 1 year of experience supporting individuals with DD/ID (preferred). Valid Kentucky driver's license with a clean driving record. Ability to pass a criminal record check, drug screen, and TB risk assessment. Strong communication, organizational, and interpersonal skills. Proficiency with computers, software, and virtual communication tools. Physical & Work Environment Requirements Ability to perform tasks such as prolonged standing, medium lifting, bending, and climbing. Comfort working in diverse environments, including outdoor and off-site locations.
