Job Summary:
The Medical Service Trainer / RN Case Manager serves a dual role as both a nurse case manager and healthcare trainer for United Cerebral Palsy of Georgia programs. This position coordinates and oversees medical services for an assigned regional caseload of individuals with intellectual and developmental disabilities while ensuring the delivery of high-quality, person-centered healthcare and support services. Utilizing nursing, case management, and adult learning principles, the RN assesses individual healthcare needs, develops and monitors plans of care, facilitates communication among interdisciplinary teams and healthcare providers, and promotes continuity of care across residential and day service settings. In addition to direct nursing and case management responsibilities, this position is responsible for developing and delivering health-related training programs for direct support professionals and other staff, including medication administration, proxy caregiving, healthcare plans, and other clinical topics. The RN provides ongoing coaching, competency validation, onboarding support, and educational resources to ensure staff are equipped to safely support individuals receiving services. The individual in this role functions as a medical case manager for an assigned group of individuals, provides direct nursing services, oversees proxy caregiving processes, reviews health and risk management tools, monitors medication and treatment compliance, participates in interdisciplinary planning, responds to emergent healthcare concerns, serves as a liaison with pharmacies and healthcare providers, and supports organizational compliance with licensing, accreditation, and regulatory requirements.

Case Manager Duties/Responsibilities:
- Maintains a working knowledge of funding, licensing and accrediting body regulations and standards related to medical services, and the internal documents that meet these requirements.
- Maintains a working knowledge of UCP of Georgia policies and procedures related to medical services, and participates in regular policy/procedure development, review, and revision.
- Regularly reviews and provides necessary approvals for Health Risk Screening Tool (HRST) documents.
- Monitors completion and results of Abnormal Involuntary Movement Scale (AIMS) testing for program participants.
- Conducts nursing case management of a caseload of individuals who participate in UCPGA programs. Tasks include but are not limited to regular program participant observation, medication check-in, pharmacy communication, provision of medical care guidance, medical appointment attendance, ISP meeting participation, hospital admission/discharge team planning, program admission/discharge team planning, proxy caregiving oversight, documentation of services, and after hours on-call availability.
- Acts as a comprehensive care coordinator for program participants by assessing individual medical needs and facilitating communication among other team members and clinicians providing care.
- Performs comprehensive assessments of physical, emotional, psychosocial, and environmental needs of program participants, including outcome information.
- Review results of laboratory tests and other diagnostic procedures and regularly report findings and recommendations to team members and other clinicians and medical professionals.
- Provides direct nursing services as needed and assigned for individuals served by UCPGA in conjunction with other agency nurses.
- Rotates weekend on-call duties with other agency nurses.
- Participates in regular case staffing, program reviews, and other meetings as necessary and assigned.
- Provides training for new employee orientation and other monthly training sessions related to medical services as needed.
Medical Service Trainer Duties/Responsibilities:
- Train new and current staff on medication administration, proxy caregiving and healthcare plans for all individuals.
- Prior to the trainings, work closely with Director of Nursing to establish class policies, processes, documentation etc.
- Teach 4-6 full sessions per month of medication administration & teach Proxy Refresher sessions.
- Work with nursing department to build positive professional relationships.
- Participate in the staff onboarding process for assigned individuals. Perform follow-up tasks for these new staff to provide feedback and to answer any potential questions after they have been released from initial training.
- While onsite/during virtual training day ensure all users have access to training materials.
- Provide content and feedback on new and revised training course materials, guides, instructional Aids and presentations.
- Provide solutions to feedback received from staff surveys related to training.
- As needed/requested, train staff in using appropriate body mechanics to lift, move and position and provides prescribed exercises and physical care to resident. Submit in-service forms to HR within 24 hours of training.
- Provide quarterly adaptive equipment monitoring/training submit in-service forms to HR within 24 hours of training.
Other Responsibilities:
- Adhere to all agency policies and procedures.
- Develop and maintain positive and productive relationships with individuals, families, co-workers, and community members.
- Maintain objectivity in position to set appropriate limits, while working with individuals and team members.
- Maintain respect for cultural, ethnic, spiritual, and individual differences.
- Travel to various program sites and external locations for meetings and other events.

- Experienced at interacting with different levels of staff within a direct support professional setting.
- Engage participants in hands-on learning. Skilled at modifying their approach to training based on the trainee’s learning style.
- Critical thinker with the ability to solve problems independently/most direct manner and recognize trends where new or enhanced training resources would improve program performance.
Must also be able to demonstrate and train clients in a virtual manner if required
Ability to work independently as well as in a team environment.
Excellent oral and written communication skills.
Exemplary planning and time management skills (this role is designed to provide onsite training and support for out direct support staff).
Required Certifications and Licenses:
RN License, CPR and First Aid Certification, Valid Georgia Driver’s License
Education and Experience:
Bachelor of Science in Nursing (BSN) preferred. A minimum of an Associate’s degree or diploma in Nursing from an accredited program is required in conjunction with a current licensure as a Registered Nurse (RN). Minimum 2 years experience of clinical nursing practice. Minimum 1 year experience in the field of developmental disabilities.
