Full Time | 8hr Shift | 10p-6:30a | $31.50/hour

Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and resident assessments

• Develop and implement individualized care plans in collaboration with the interdisciplinary care team

• Maintain accurate and up-to-date records of resident care, including progress notes and medication administration records

• Collaborate with other healthcare professionals to ensure seamless transitions and high-quality care

• Participate in quality assessment and improvement activities to ensure compliance with regulatory standards

• Maintain a clean and organized work environment, adhering to infection control guidelines

Requirements:

• Current LPN licensure in the State of Ohio

• Minimum 0 years of experience in a long-term care setting

• High school diploma or equivalent required; ADN or diploma in nursing preferred
