Supportive Leadership Collaborative In-House Therapy Team Opportunities for Professional Growth Meaningful Work with a Resident-Centered Focus What You’ll Do Evaluate residents and develop individualized plans of care Provide skilled physical therapy services based on each resident’s needs and goals Help residents improve strength, balance, mobility, and functional independence Monitor progress and adjust treatment plans as appropriate Collaborate with nursing, physicians, therapy staff, residents, and families Complete accurate and timely clinical documentation Maintain compliance with applicable state and federal requirements Qualifications Current Illinois Physical Therapist (PT) license Strong communication and interpersonal skills Compassionate and resident-focused approach Ability to work effectively as part of an interdisciplinary team Skilled nursing or rehabilitation experience preferred, but not required New graduates are encouraged to apply!
