Responsibilities
- Administer prescribed nursing care and medications per physician orders under RN supervision
- Monitor and document patient status, including vital signs and responses to treatments
- Assist with activities of daily living and provide comfort measures to residents
- Perform routine wound care, dressing changes, and basic clinical procedures as directed
- Maintain accurate documentation in the medical record and ensure compliance with privacy and safety standards
- Communicate effectively with the interdisciplinary team and escalate changes in patient condition to the supervising RN

Requirements
- Active New York State Licensed Practical Nurse license
- BLS certification
- Minimum 2 years of experience in a long-term care or skilled nursing setting
- Education: completion of an approved practical nursing program
- Ability to work 5x8 night shifts; every other weekend required; must be able to work at least 4/8s
- Willingness to travel; local travelers within 50 miles accepted with a decreased bill rate
