Family Case Manager

Wellmore Behavioral Health

Wellmore Behavioral Health in Waterbury, CT seeks a full-time Case Manager to provide services to families referred by DCF. You will conduct intake assessments, develop individualized care plans, coordinate housing and community resources, and connect families to medical, behavioral health, financial and social services. You will monitor progress through home visits, collaborate with DCF social workers to promote child safety and family well-being, maintain confidential records, prepare reports, #J-18808-Ljbffr
