Physical Therapist (PT) – Full-Time
Civita Care Center at Cheshire
Cheshire, CT
$45–$50/hour

Post Rehab is seeking a Physical Therapist (PT) to join our rehabilitation team. This position offers the opportunity to work with residents in both short-term rehabilitation and long-term care while helping them improve mobility, strength, balance, and overall function.

Position Responsibilities:

• Perform physical therapy evaluations and establish individualized treatment plans
• Provide treatment focused on mobility, gait training, transfers, balance, strengthening, endurance, and pain management
• Collaborate with PTA staff, nursing professionals, physicians, and interdisciplinary team members
• Monitor resident progress and adjust treatment plans as appropriate
• Educate residents and families on exercises, mobility techniques, and safety recommendations
• Maintain accurate and timely documentation

Qualifications:

• Active Physical Therapist license required
• Strong communication and clinical reasoning skills
• Skilled nursing experience preferred but not required
