Patient Population: Adult, Geriatric

Setting: Primary Care Office

Responsibilities:

Adult outpatient care
Evaluation and management of acute volume/BP issues
Employment physicals
Disability exams
EKG interpretation

Required Procedures:

EMR System: Athena

Support Staff:

5 Physicians
1 Mid-Level Provider

On-Call/Weekends: None

Submission Requirements:

New Jersey State License Required
Local Provider Required
Self-Query NPDB Report (within 30 days) Required
Board Certification Preferred
