Registered Nurse Acute Care

Role Overview: The RN Complex Care Coordinator assesses and manages care for complex patients, ensuring safe and effective discharge planning across the continuum. Key Responsibilities: Conducts comprehensive clinical and psychosocial assessments, develops individualized discharge plans, and collaborates with interdisciplinary teams to coordinate community resources, post-acute care, and social services. Provides guidance on psychosocial, legal, and resource needs, including end-of-life planning. Acts as a resource for care team members and mentors new staff. Facilitates hospital meetings, advocates for patients, and supports community agency negotiations. Maintains accurate documentation and participates in professional development and protocol development. Qualifications & Skills: Minimum 3 years healthcare experience, with at least 2 years in care coordination in an acute care setting. Strong clinical, social, and resource management expertise. Excellent communication, assessment, and mentoring abilities. Ability to handle complex psychosocial and discharge challenges, and work collaboratively to optimize patient outcomes. #J-18808-Ljbffr
