Organization: ANIZ, Inc.
Reports To: Clinical Director
Salary: $40,000 annually

Position Summary

ANIZ, Inc. is seeking a compassionate, organized, and detail-oriented SAMHSA & Ryan White Case Manager to support client engagement, service coordination, program tracking, and retention efforts. This position works closely with clinical, Outreach, and data teams to ensure clients receive timely services, remain engaged in care, and that program goals and reporting requirements are met.

Key Responsibilities

• Coordinate client intake appointments and assist with engagement and retention efforts for new, active, and re-engaging clients.
• Monitor client progress to ensure required services, follow-ups, surveys, and program milestones are completed.
• Maintain accurate client tracking systems, spreadsheets, and electronic records.
• Conduct outreach to clients who miss appointments and assist with re-engagement into care and supportive services.
• Educate clients on available resources, treatment adherence, eligibility requirements, and the importance of ongoing care.
• Serve as a liaison between clients and multidisciplinary teams to support service coordination and continuity of care.
• Track program performance, identify service gaps, and assist with monthly and quarterly reporting requirements.
• Support data quality and compliance activities by ensuring documentation is complete, accurate, and aligned with SAMHSA and Ryan White requirements.
• Participate in quality improvement initiatives, team meetings, support groups, and client engagement activities.
• Assist with administrative duties, including documentation collection, scanning, reporting, and maintaining client records.

Qualifications

• Bachelor’s degree in social work , Public Health, Psychology, Human Services, or a related field required .
• Master’s degree preferred .
• Experience working in behavioral health, public health, HIV services, case management, or community-based programs preferred.
• Knowledge of trauma-informed care, client engagement, and service coordination practices.
• Strong organizational, communication, and time-management skills.
• Proficiency with electronic records systems, Microsoft Office, Excel, and data tracking tools.
• Ability to work independently while collaborating effectively within a multidisciplinary team.
• Commitment to maintaining confidentiality and providing culturally responsive services.
