Interim Social Services Director

We are seeking an experienced and compassionate Interim Social Services Director to join our team in Somerset, PA. This leadership role is responsible for overseeing the social services department and ensuring the emotional, psychosocial, and discharge planning needs of our residents are met with professionalism, dignity, and compassion. The ideal candidate will have experience in a skilled nursing or long-term care setting and be comfortable stepping into a fast-paced environment while providing support to residents, families, and interdisciplinary teams. Responsibilities Oversee and manage the Social Services Department on an interim basis Complete psychosocial assessments and care plans for residents Assist residents and families with adjustment, emotional support, and care transitions Coordinate discharge planning and community resource referrals Advocate for resident rights and ensure resident concerns are addressed promptly Participate in care plan meetings and interdisciplinary team discussions Maintain accurate and timely documentation in accordance with state and federal regulations Support admissions, transfers, and discharge processes Ensure compliance with all applicable healthcare regulations and facility policies Qualifications Bachelor’s degree in Social Work, Human Services, or related field preferred Previous experience in long-term care or skilled nursing strongly preferred Prior leadership or supervisory experience preferred Knowledge of state and federal regulations related to social services in healthcare Strong communication, organizational, and problem-solving skills Ability to work collaboratively with residents, families, and healthcare teams Benefits Competitive pay Supportive team environment Opportunity to make a meaningful impact in resident care Flexible interim assignment opportunity We are looking for a dedicated professional who is passionate about improving the quality of life for residents and supporting families through every stage of care.
