Responsibilities
- Collect data on residents’ physical and psychosocial condition and report findings to the RN and the Interdisciplinary Team.
- Contribute to the development of an age-appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions; involve resident/family and healthcare team.
- Revise the plan of care as outcomes are evaluated and referrals are indicated to address all aspects of resident care.
- Provide direct and indirect skilled nursing care under the supervision of a Registered Nurse; maintain a safe, clean, and collaborative care environment.
- Collaborate with physicians, other nursing staff, and ancillary personnel to coordinate care and ensure resident needs are met.
- Support, mentor, and orient staff as appropriate; maintain current knowledge and competency through continuing education and in-service programs.
- Demonstrate professional practice leadership under the direction of the RN and adhere to safety and infection control standards.

Requirements
- Active NY Licensed Practical Nurse license
- BLS certification (American Heart Association or RQI)
- Minimum 1 year of LPN experience in a long-term care facility
- Ability to work evenings (5x8 schedule) and adapt to the geriatric population
- Ability to function effectively under RN supervision within a multidisciplinary team
