Licensed Practical Nurse

Job Summary Provide nursing service, documentation and record review. Oversee medical information; verify follow up and documentation; chart reviews, appointments. Duties & Responsibilities The LPN's essential duties and responsibilities include the following. Other duties may be assigned to meet the needs of the individuals and the program. Ensure documentation is accurate with follow up noted Complete/ensure completion of routine work including but not limited to; doctors orders, follow up appointments/tests/specialists, maintain tickler, face sheets, BM charts, Braden scales, self-medication checklist, schedule appointments as needed Provide bedside care Administer medication and document therapeutic effects Take vital signs (temperature, blood pressure, pulse and respiration) Assist with resident comfort Collect samples Provide wound care Help coach staff as necessary Make and follow up on Dr, orders, appointments, etc. Education & Experience HS diploma 2+ years experience Up to date first aid and CPR certification Current LPN license in the state Exceptional staff training and customer service skills Compassionate and friendly demeanor Physical Requirements While performing the duties of this job, the employee is regularly required to communicate clearly and effectively with staff and consumers.
