About the Role:

We are seeking a dedicated and compassionate LPN to join our team at our Burlington, New Jersey location. As an LPN at Silver Healthcare, you will be responsible for providing high-quality, individualized care to our patients, working closely with our interdisciplinary team to develop and implement personalized care plans. You will also be responsible for administering medications, performing vital signs, and reporting changes in patient condition to our medical staff.

Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and reporting changes in patient condition

• Develop and implement personalized care plans in collaboration with our interdisciplinary team

• Assist with daily living activities, such as bathing, dressing, and grooming

• Maintain accurate and detailed records of patient care and treatment

• Participate in shift reports and communicate effectively with other members of the healthcare team

• Provide emotional support and empathy to patients and their families

Requirements:

• Current LPN licensure in the state of New Jersey

• 0-1 year of experience in a long-term care setting or related field
