Job Overview The Transition Facilitator develops and implements transition programs focused on the social, intellectual, and vocational growth of youth with disabilities at Mesa Public Schools. This position coordinates career counseling and career development services for youth in junior high and high schools to ensure a seamless transition from school to the adult world. This position adheres to the teacher salary placement and teacher calendar. Employment Type: Full-time Base Salary: $55,886.00 Calendar: Certified Exempt, 187 workdays Benefits: Sick leave, holidays, matched retirement through the Arizona State Retirement System, health, and life insurance Performance Pay: Available annually New Hire Experience: $500 for each full-time year of certified teaching or verified like experience New Hire Education: $65/hour of graduate level credits: BA +45 hours max, MA +60 hours max; $1000 for advanced degrees Qualifications & Requirements Required Qualifications: Bachelor’s degree in vocational evaluation, counseling, special education, psychology, transition, or related field Master’s degree (Preferred) in vocational evaluation, counseling, special education, psychology, transition, or related field Valid Arizona Special Education teaching certificate Five years experience in special education, vocational training, or related area providing services to youth with disabilities Experience in aligning special education and general education systems (curriculum, assessment, etc.) for student-driven transition outcomes Skills & Competencies: Models the attributes and skills of the MPS Portrait of a Graduate Demonstrates commitment to the MPS Promise that every student is known by name, served by strength and need, and graduates ready for college, career and community Ability to instruct in a typical classroom environment as well as within the community career exploration, on-the-job training, and travel training skills Professional training and experience in vocational evaluation, rehabilitation counseling, special education teaching or areas related to results-based education Knowledge of school-to-careers transition planning, supported employment, competitive employment, and career and technical education options Ability to meet deadlines, work on multiple projects, coordinate work with others, and be self-directed Ability to reflect on student outcomes and effective transition practices Knowledge of components of teaching self-advocacy and self-determination skills to any youth with a disability Ability to develop and maintain cooperative and effective professional relationships Ability to communicate effectively (verbal and written) with diverse community members (e.g. youth, families, educators, adult service providers, employers) to help promote seamless transition services for youth with disabilities Work Environment & Physical Requirements Office and classroom setting within the district or on a school campus May travel to schools, homes, job sites, or community centers to meet clients and coordinate resources Frequently interacts with clients, families, educators, employers, and service providers May require evening or weekend meetings to accommodate client availability May require walking, standing, or traveling between different locations Prolonged sitting for administrative tasks, documentation, and meetings Occasionally you may need to transport documents, equipment, or assistive devices up to 20 lbs Equal Opportunity Employer Statement Mesa Public Schools is an equal opportunity employer and complies with all applicable federal, state, and local laws regarding equal employment opportunity and anti-discrimination.
